
Eldercare Planning and Guidance is the newest service being offered by SAGE
Eldercare. Eldercare Planning and Guidance assists older adults and their caregivers by
assessing individual needs, creating a plan of care to meet those needs and offering
guidance through the system of services available to seniors. The goal of Eldercare
Planning and Guidance is to work with families and other care providers to ensure that seniors receive services that support their health and well-being, maximize
functionality, decrease isolation and allow them to remain living independently as long as possible.
Eldercare Planning and Guidance is coordinated by a Registered Nurse with
extensive experience in geriatric care management who provides in-home
support through education and consultation directly to older adults and/or those involved in the provision of their care. Assistance includes a comprehensive needs assessment along with an individualized care plan to meet those needs. The plan of care includes guidance and referrals to community resources and coordination with local agencies. Follow-up home visits and health monitoring are provided as needed. Caregivers receive ongoing personal support and resources throughout their involvement with the Program.
